WORK HOURS: Monday – Friday, 8:30 a.m. – 5:00 p.m.
GENERAL
STATEMENT OF DUTIES: Responsible
for providing comprehensive case management services, linkage, coordination of
resources, and monitoring of services provided to individuals who meet criteria
for serious mental illness services.
Exercise sound judgment, demonstrate initiative, and maintain
confidentiality in accordance with established policies and procedures.
ESSENTIAL
FUNCTIONS: Responsible for consumer assessment, service linkage,
coordination, development, and implementation of case management individual
service plans. Assist and provide follow
up instruction, education, and counseling to consumers/families with SMI. Monitor
service delivery through contacts with consumers receiving services, family
members, service providers, and periodic site/home visits. Manage consumer
records according to Licensure and Medicaid standards.
QUALIFICATIONS: BA/BS in Psychology,
Social Work, Counseling, or related Human Services field; one year direct care
clinical experience with SMI individuals; must meet QMHP eligibility requirements;
valid Driver’s License with an
acceptable DMV record.
