WORK SITE:                Franklin/Suffolk/Windsor/Smithfield

 

WORK HOURS:       Monday – Friday; 8:00 a.m. – 4:00 p.m.

Work Schedule May Flex to Accommodate Program Needs

 

GENERAL STATEMENT OF DUTIES:  Provide therapeutic behavioral support to child/adolescent consumers to assist them in increasing functional life skills and deter behaviors that could cause out-of-home placement.  Provide quality care and assist consumers in achieving optimal potential in the least restrictive environment. 

 

ESSENTIAL FUNCTIONS:  Responsible for developing and implementing individualized behavior management and modification programs.  Maintain daily point sheets and document all relative information related to consumers.  Co-facilitate psycho-educational and skills building activities.  Advocate for the consumer and the consumer’s family.  Establish and maintain inter-agency and intra-agency relationships regarding provision of needed services.

 

QUALIFICATIONS:  BA/BS in a Human Services field; one year clinical experience working directly with SMI or SED individuals; must meet QMHP requirements, and possess a valid Driver’s License with acceptable DMV record at the time of employment and continuously thereafter.
