GENERAL STATEMENT OF DUTIES: Provide
therapeutic behavioral support to consumers to assist them in increasing
functional life skills and deter behaviors that could cause out-of-home
placement. Provide quality care and
assistance to consumers in achieving optimal potential in the least restrictive
environment possible.
ESSENTIAL FUNCTIONS: Serve
as a member of the Therapeutic Day Treatment team. Responsible for coordinating
admission procedures, developing and implementing individualized behavior
modification/ management programs.
Maintain and document daily point sheets and all other relative
information related to consumers.
Establish and maintain inter-agency relationships regarding provision of
needed services and serve as an advocate for the consumer’s family.
QUALIFICATIONS: BA/BS
in a Human Services field; one (1) year experience working directly with SMI or
SED individuals; must be QMHP.
HIRING SALARY: $18.00 per Hour.
