What Is Case Management?

Case managers are healthcare professionals who serve as advocates for our community members. They support, guide and coordinate care for clients, and their families and caregivers, throughout their wellness journey.

Case management is an important part of the treatment process. Case managers help individuals to navigate our complex healthcare system. They help to identify the services, providers and supports that will best meet the client’s needs and promote client wellness and autonomy through communication, advocacy and education.

Case Management Services Include:

  • Medical
  • Psychiatric
  • Social
  • Educational
  • Vocational
  • Other supports essential to meeting basic needs

Western Tidewater Community Services Board (WTCSB) offers Case Management services for individuals residing in the WTCSB service area (the cities of Franklin and Suffolk and the counties of Isle of Wight and Southampton), including:

  • Adults experiencing mental health concerns
  • Adults with Intellectual or Developmental Disorder
  • Children and teens experiencing mental health concerns
  • Children and teens with Intellectual or Developmental Disorder
  • Individuals seeking help for substance abuse

Case Management ensures that adults with mental health issues and/or intellectual disabilities can access the full range of community supports that are available to them and their families.
