JOB TITLE: COUNSELOR I,

WORK SITES: Tidewater House-Suffolk (2 Positions)

Will vary based on program needs and will include
evenings
GENERAL STATEMENT OF DUTIES: Provide comprehensive psychosocial rehabilitation and
case management services to individuals who are suffering from serious mental
illness, substance abuse and/or are dually diagnosed.
ESSENTIAL FUNCTIONS: Assist with consumer service plans related to
identified needs and goals; transport and assist consumers in gaining access to
appropriate medical, psychiatric, social, educational, vocational and other
supports and services; provide consumer training/reinforcement, provide linkage
to supports/services; advocate on behalf of the consumer, family and providers
for the purpose of gaining access to services; monitor/assess service delivery;
provide behavior management and crisis intervention services.
QUALIFICATIONS: Bachelor’s Degree in Human Services (including but not
limited to Social Work, Psychology, Counseling, or equivalent). Master’s Degree is preferred; two (2) years
experience working directly with adults with mental illness, mental
retardation, substance abuse, and/or dual diagnosis; Qualified Mental Health
Professional, (QMHP); valid Driver’s License and acceptable driving record; personal
transportation for on-the-job use as needed.
