JOB TITLE:          COUNSELOR II,  

                                Mental Health Support Services (School Based)

           

POSITION:                0752

 

WORK SITE:        Smithfield, Virginia

                       

WORK HOURS:  7:00A.M. – 3:00P.M.

 

GENERAL STATEMENT OF DUTIES:  Provide high school based counseling, comprehensive case management and service coordination primarily to adolescents diagnosed with SED.

 

ESSENTIAL FUNCTIONS:  Provide training, education, and/or reinforcement of consumer’s functional skills and activities of daily living. Provide advocacy and support services, social integration, and use of community resources.  Monitor consumers and provide assistance with behavior management, nutrition, health, and overall physical condition. 

 

QUALIFICATIONS:  BA/BS Degree in Human Services field; two year (2) clinical experience in a mental health, substance abuse or similar setting; must be a Qualified Mental Health Professional, QMHP, or meet eligibility requirements; valid Driver’s License with acceptable DMV record at the time of employment and continuously, thereafter.
