JOB TITLE:               Counselor II, MR Residential Services

WORK SITE:            Zuni, Virginia (TNP)

 

WORK HOURS:       Suffolk Schedule: 

                                                Week 1:  Mon– Fri – 1- 9::00 p.m.; Sat-Sun- Off

                                                Week 2:  Mon-Wed –1- 9::00 p.m.; Thu  & Fri - off;  Sat & Sun 8a-4:00 p.m.

 

GENERAL STATEMENT OF DUTIES:  Responsible for supervision and evaluation of consumers, the development and implementation of consumer treatment plans, care and maintenance of a safe environment.  Responsibility also includes supervision, training, and evaluating subordinate staff.  

 

ESSENTIAL FUNCTIONS:  Ensure proper implementation of consumer ISP’s; perform monthly review of each consumer’s progress as noted on IPP; assist Social Worker with consumer/family interactions; perform monthly review of each consumers progress, etc. as noted on IPP; ensure accurate completion of data collection; ensure accurate implementation of IPP; staff training and development; assist Program Supervisor with updating formal assessment devices; monitor and evaluate effectiveness of shifts; audit face sheets monthly for content and accuracy.

 

QUALIFICATIONS:  BA/BS Degree in Human Services field; two years direct care service to individuals diagnosed with Mental Retardation or other developmental disabilities; must be QMRP (or eligible within 90 days of hire date); valid Driver’s License with acceptable DMV.
