2008 Golf Tournament Registration Form

Please join us for our Annual Western Tidewater Community Services Board Golf Tournament! 

We will be playing at Cypress Creek Golfers' club in Smithfield, VA on Friday, September 19, 2008. 

Entry fee:  $85.00 per person/$320.00 per team

1:00pm Shotgun Start

Warm-up 11:30am -12:00pm

Fee includes Green Fees, Cart, Range Balls and Lunch

Contests!  Prizes! Many Give-a-ways!
