WORK HOURS: Monday – Friday
8:30 a.m. – 5:00 p.m.
GENERAL STATEMENT OF DUTIES: Provide comprehensive case management and services coordination to individuals who are Intellectually Disabled (I.D).
ESSENTIAL FUNCTIONS: Responsible for consumer assessment, service linkage, coordination, promotes and assists with development and implementation of the Individual's Individual Service Plan, ISP. Provide follow up instruction, education, and counseling to consumers/families. Monitor service delivery with consumers, family members,and service providers; provide periodic site/home visits.
QUALIFICATIONS: Bachelor’s Degree in Human Services; one year clinical experience;must be QMHP/QMRP;computer proficiency; valid Driver’s License with an acceptable DMV record.
