WORK SCHEDULE: Work as available/work as needed
GENERAL STATEMENT OF DUTIES: Provide counseling, comprehensive case management and service coordination primarily to adult individuals with SMI.
ESSENTIAL FUNCTIONS: Provide training, education, and/or reinforcement of consumer’s functional skills and activities of daily living. Provide advocacy and support services, social integration, and use of community resources. Monitor consumers and provide assistance with medication management, nutrition, health, and overall physical condition.
QUALIFICATIONS: BA/BS Degree in Human Services field; one (1) year clinical experience in a mental health, substance abuse or similar setting; must be a Qualified Mental Health Professional, QMHP; valid Driver’s License with acceptable DMV record required.
