Western Tidewater Community Services Board's philosophy of Case Management is based on the ethical treatment and the values of self-determination, positive regard, dignity and respect for consumers with Mental Health, Intellectual Disabilities, and Substance Abuse disabilities from diverse cultural and ethnic backgrounds. Our purpose is to be the central point of contact for consumers seeking public mental health, mental retardation and substance abuse services. It is out goal to assess the needs of consumers seeking services and provide ease of access to those services deemed appropriate. Our main objective is to assist consumers in maintaining a satisfactory quality of life while remaining integrated in the community.

 

 
Case Management Services include:
  • Planning and assessment
  • Linking the individuals to services and supports
  • Assisting with developing and/or obtaining needed supports and resources
  • Coordination of services and service planning
  • Contacting providers or resources
  • Making collateral contacts with significant others
  • Monitoring progress and service delivery
  • Counseling and education
  • Advocacy
