Mental Health Case Manager

Western Tidewater CSB is seeking qualified Case Managers for various departments throughout our organization in our Suffolk and Franklin locations.

**Substance Abuse, Mental Health, or ID/DD experience is preferred for this position. **

-Outpatient/Adult

-School Counseling Intervention Program

-Franklin Outpatient

-Permanent Support Housing

Responsibilities and Duties for Case Manager:
  • Responsible for consumer assessment, service linkage, coordination, development, and implementation of a case management ISP

  • Assist and provide follow up instruction, education, and counseling to consumers/families with SMI/SA/SED/ID/DD

  • Monitor service delivery through contacts with consumers receiving services, family members, service providers, and periodic site/home visits

  • Manage consumer records according to all Licensure and Medicaid standards.

Qualifications and Skills for Case Manager:
  • BA/BS from an accredited college or university with major course experience in Psychology, Social Work, or related field

  • One year of clinical experience in a Human Services field

  • QMHP or QIDP preferred

  • KSA’s (Required)

  • Valid Driver’s License with an acceptable DMV record at the time of employment and continuously thereafter
