Case Management
It’s hard enough living with a mental health condition, but when you have to navigate our complex healthcare system to get the help you need, it seems impossible.
What Is Case Management?
Case managers are healthcare professionals who serve as advocates for our community members. They support, guide and coordinate care for clients, and their families and caregivers, throughout their wellness journey.
Case management is an important part of the treatment process. Case managers help individuals to navigate our complex healthcare system.
