A safe, home-like environment for stabilization and healing

When an adult with developmental disabilities experiences behavioral instability, finding the right care can feel overwhelming. WTCSB’s REACH CTH provides a highly supportive, 24/7 residential setting designed to assess needs, create a plan of care, and help individuals stabilize safely. We believe that every individual should be supported to live the most inclusive life possible in their community. By providing prompt, compassionate treatment and prevention, we help individuals safely transition back to their daily routines without the need for hospitalization.

What the CTH is

The CTH is a regional 6-bed, short-term residential program. It acts as a comfortable, structured intervention where a multidisciplinary team—including counselors, psychiatrists, registered nurses, behaviorists, and direct support professionals—provide round-the-clock care.

During a stay at the CTH, our team will “walk alongside” the individual to provide:

  • Assistance with Activities of Daily Living (ADLs) and community integration.
  • Therapeutic groups, individual counseling, and de-escalation support.
  • Ongoing nursing support and daily medication management, including arranging updated prescriptions to a designated pharmacy.
  • Development of a personalized Crisis Education and Prevention Plan (CEPP) to help families and providers better understand behavioral needs and support community stabilization.
  • Weekly Treatment Team meetings with natural and community supports.
  • Care coordination with the REACH Mobile Team and case management linkage to community care and supports.

Who the CTH serves

The CTH program serves adults (ages 18 and older) who have intellectual and developmental diagnoses along with co-occurring mental health diagnoses. 

Our short-term residential services can support adults who are:

  • In need of short-term prevention (typically a 5-7 day stay).
  • Stepping down from hospitalization or other inpatient setting for a transition period (typically a 14-day step-down stay).
  • Experiencing an exacerbation of behaviors that has not been successfully managed in their current home(up to a 30-day stay).

How to Access Care at the CTH?

Because the CTH provides highly specialized residential care, admission is often coordinated through our crisis teams, hospitals, or community providers. However, reaching out to start the process is simple.

Provider Referrals: Clinicians, hospitals, case managers, and community partners looking to refer an individual for step-down or prevention services can contact our team directly at admissions@wtcsb.org.

How to Pay for My Visit

WTCSB accepts public insurance, private insurance, and self-pay. If you have insurance, we will use your coverage when eligible and let you know if any copays or costs apply. WTCSB’s published payment options include Virginia Medicaid (including CCC Plus plans), Medicare, and many commercial insurance plans.

If you do not have insurance, or if you are underinsured, WTCSB also offers income-based financial assistance through an Ability to Pay (ATP) system (often called a sliding scale). Fees are based on what you can afford, and some services may have different fee rules than others.

The CTH is a secure, 6-bed residential facility located in Hampton, VA.

Because this is a 24/7 specialized care home rather than a walk-in community clinic, exact location details and transportation are coordinated directly with individuals, families, and providers upon admission.
